Almost always one of three things: documentation (missing signatures, wrong form version, incomplete packets), timing (training started before the contract was executed, or a late submission), or financial (the claim exceeds the program cap, or payroll records don't match the amount claimed).

Opportunities we only identify for your CPA to execute carry no fee.

Never on self-serve plans. Flat membership plus flat filing fees is the whole model. Our Managed tier for enterprise operators uses a platform fee plus a performance share above a committed floor, agreed in writing before any work begins.
