DEN-001 — Missing supervisor signature on attestation. Fix: Obtain signed attestation before resubmission. Must include printed name, signature, and date. DEN-002 — Missing or incomplete training plan. Fix: Create training plan with occupation, hours, milestones, and both employer + case manager signatures. DEN-003 — Agreement date is after training start date. Fix: OJT agreement must be executed BEFORE training begins. No retroactive contracts allowed under TWC policy. DEN-004 — Claimed wage exceeds program cap. Fix: Adjust claimed amount to program maximum hourly rate. Check board-specific caps which may differ from TWC default.
